最低抑菌浓度漂移现象研究现状

耐甲氧西林金黄色葡萄球菌(methicillin-resistant Staphylococcus aureus, MRSA)是医院及社区获得性感染常见病原体,万古霉素是其治疗一线用药。近些年来,万古霉素治疗MRSA感染最低抑菌浓度(MIC)出现的漂移现象(MIC creep)[1-2]引起大家广泛关注。本文就一些基本概念、MIC漂移现象研究现状及MIC与临床疗效的关系等综述如下。

1 MRSA相关概念

MRSA是指对苯唑西林耐药的金黄色葡萄球菌,多为医院获得性感染[3]。其于1961年首次在英国发现[4]。2017年全国细菌耐药监测网显示我国西藏地区MRSA检出率高达52%。该微生物已成为严重威胁人类健康的“超级细菌”。

根据感染场所不同,MRSA可分为医院获得性MRSA(hospital-acquaired MRSA, HA-MRSA)及社区获得性MRSA(community–acquired MRSA, CA-MRSA)。HA-MRSA对多种抗菌药物耐药,分子检测多为SCCmec Ⅰ-Ⅲ型[5]。CA-MRSA是分离自社区感染者的一种新型菌株,其仅对β内酰胺类抗菌药物耐药,分子检测多为SCCmec Ⅳ-Ⅴ型[5]。然而近些年来,CA-MRSA发病率越来越高,耐药性日趋明显,基因型和HA-MRSA交叉显著,两者之间的区分比较困难[5-6]。鉴于此,MRSA也可分为医院起始MRSA(hospital-onset MRSA, HO-MRSA)和社区起始MRSA(community-onset MRSA, CO-MRSA)[6]

根据对万古霉素的敏感性不同,金黄色葡萄球菌又可分为敏感、中介耐药与异质中介耐药、耐药金黄色葡萄球菌(vancomycin susceptible S. aureus, VSSA、vancomycin intermediate S. aureus, VISA、heteroresistant VISA, hVISA、vancomycin resistant S. aureus, VRSA)。根据2012年版CLSI(clinical and liboratory standards Institute, CLSI)标准定义[7],MIC≥16 mg/L为VRSA,MIC在4~8 mg/L为VISA,MIC≤2 mg/L为VSSA。

hVISA是指母代对万古霉素敏感(MIC≤2 mg/L),而子代却含有少量(10-6)能够在≥4 mg/L的万古霉素培养基上生长的耐药亚群[8]。其于1997年首先在日本发现[2, 9]。hVISA检测较为困难, 各种检测方法各有利弊,目前尚无省时且准确的检测方法[10-11]。Saito等[12]发现一种新型hVISA:sVISA(slow VISA),其生长较传统hVISA缓慢,但MIC值能达到8 mg/L,甚至更高。

VRSA较少,美国共发现13株[13]。其耐药原因是从VRE(vancomycin-resistant enterococci,VRE)获得了vanA基因[13],其余耐药机制不明。VISA/hVISA与VRSA不同,前者不含vanA基因,其耐药机制不明。Gardete等[14]发现,VISA/hVISA存在walkR、vraSR、rpoB等不同位点基因突变,大部分突变位点和细胞壁的合成或代谢相关,这些突变的出现与糖肽类抗生素的大量使用相关,但与耐药机制的关系尚不清楚。

2 MIC检测方法

MIC表示抑制细菌生长所需的最低药物浓度[15]。对于万古霉素MIC的检测,不同方法所得结果略有差异,目前常用的检测方法包括非自动化的纸片扩散法、Etest法、稀释法及自动化的the MicroScan、VITEK-2、Phoenix automated systems等。

由于纸片扩散法不能很好地反映万古霉素实际药敏试验情况,因此CLSI推荐该法仅对检测VRSA可靠;Etest法基于纸片扩散法,其测定结果常比稀释法高1~2个稀释度;稀释法包括宏量、微量及琼脂稀释法,均是CLSI推荐的方法[7]

The MicroScan及Phoenix automated systems均采用微量肉汤稀释法通过测定液体浊度检测MIC,但前者需培养24 h得到结果,后者仅需6~16 h[16]。VITEK-2则是在某一固定的短暂时间内连续检测药敏试验卡片上细菌生长情况,通过与对照组生长曲线对比从而推测出其MIC [16]

Chen等[16]对393例MRSA菌株同时使用琼脂稀释法、Etest、the MicroScan、VITEK-2、Phoenix automated systems五种方法检测发现,高MIC(≥ 2 mg/L)菌株所占比例依次是14.2%、9.7%、28.8%、22.6%及3.1%;非自动化方法检出的高MIC是增加病死率的独立危险因素,自动化检测方法则无此预测作用;相对于琼脂稀释法,Etest法对治疗结果有更好的预测作用。有研究发现Etest法检测结果是稀释法所得结果的2倍,但当稀释法所得结果为2 mg/L或以上时,其与Etest法检测结果相当,甚至低0.5 mg/L[17-19]。Himani等[20]基本赞同上述观点,但否认了Etest法检测结果是稀释法所得结果的2倍,并指出相对于VITEK-2而言,Etest法和稀释法具有更好吻合性。Muñoz-Cobo等[21]指出Phoenix automated system检测结果较Etest法低, 并对其用于检测hVISA产生质疑。与VITEK-2及微量稀释法相比,Etest法能够更准确地反映临床效果[22-23]

目前普遍认为非自动化检测方法较自动化检测方法准确性高。CLSI推荐的微量肉汤稀释法为标准方法,但因其费时、费力且对技术水平要求较高而不被一般实验室常规采用[24]。Etest法因其敏感度高、操作相对简单而被广泛采用。

3 MIC漂移现象

MIC漂移指不同时期分离的金黄色葡萄球菌虽然仍在抗菌药物(主要指万古霉素)敏感范围内,但其MIC出现逐渐上升的现象[25],该现象在众多文献中均有报道。

钟南山院士团队用微量肉汤稀释法测定2006—2011年1 411份MRSA万古霉素MIC以评价我国MIC漂移情况,结果显示,所有菌株对万古霉素敏感,其MIC几何均数呈逐年上涨趋势(P < 0.01),MIC位于1~2 mg/L的菌株百分比也呈逐年上涨趋势(P < 0.01),但该研究67%的标本来自下呼吸道[26]。Yeh等[27]用Etest法检测中国台北2001年、2005年、2009年140例血流感染MRSA菌株万古霉素MIC并计算几何均数发现,近10年该值有明显上升趋势,从而证实血液标本中也存在MIC漂移现象。Ho等[28]证实中国香港地区MRSA血流感染中也存在MIC漂移现象,并认为菌落CC8/SCCmec ⅢⅢA(agr group Ⅰ)、CC45/SCCmec Ⅳ/Ⅴ(agr group Ⅳ)及其他少见菌落的MIC随时间增加导致MIC漂移现象产生。该机构2000—2008年万古霉素使用密度从58.4日标准计量/10 000住院日升至96.2日标准计量/10 000住院日(P < 0.01),提示可能是导致MIC漂移的一个原因。我国否认MIC漂移现象的报道较少,但同样位于亚洲的印度未发现MIC漂移现象[29]

Kehrmann等[30]用Etest法检测德国287例血流感染MRSA万古霉素MIC,结果显示某城市发现MIC漂移现象,但另一城市未发现,从而提出MIC漂移现象可能存在地域分布。Steinkraus等[31]对美国一家医院662例血培养分离的MRSA研究发现,万古霉素MIC从2001年0.62 mg/L升至2005年0.94 mg/L,而来自美国同一时期不同地区的另一项研究却并未发现MIC漂移现象,从而支持MIC漂移现象可能存在地域分布[32]

南加利福尼亚大学实验人员用微量稀释法检测299例MRSA标本MIC发现,29年间该值无明显上涨趋势[33]。来自英国的研究显示MIC > 0.75 mg/L(琼脂稀释法)的菌株呈逐年上升趋势,但差异无统计学意义,不除外与样本量较小相关[34]。来自西班牙[35]及葡萄牙[25]的研究也未发现MIC漂移现象。

Reynolds等[36]收集2001—2007年英国19家实验室标本,并在一周内用琼脂微量扩散法再次检测发现:原始数据呈明显上升趋势,但复测结果呈缓慢下降趋势,出现该现象的原因可能与媒介,操作人员误差,抗菌药物活性变化等相关,而EMRSA-16(MIC几何均数为0.81 mg/L)从21%降至9%,EMRSA-15(MIC几何均数为0.74 mg/L)从76%升至85%,也可能是一原因。因不同时期对同一标本检测结果不同,应用历史数据研究MIC漂移可能呈现假性升高,因此该时期英国并未发现MIC漂移现象。

关于MIC漂移现象是否存在尚有争议,以下原因可能导致研究结果产生差异:

⑴检测方法的不同。不同的检测方法敏感性不同,对hVISA的识别率也不同,理论上会影响最终检测结果。但目前关于不同检测方法所致MIC漂移现象差别的研究较少。

⑵分析方法的不同。Sader等[37]对2002—2006年美国9家医院MRSA菌株研究发现,当采用平均数分析数据时,万古霉素MIC逐年无明显变化。但采用几何均数分析数据时,三家医院MIC呈上升趋势,三家医院呈下降趋势,故不同分析方法可能是造成不同研究结果不一致的原因。

⑶检测时间不同。Edwards等[38]用Etest法分别检测同一菌株刚分离时及冷冻保存五年后的MIC,结果显示后者远低于前者,从而证实冻存法储存菌株会对MRSA的MIC产生影响。Reynolds等[36]进一步证实检测时间对检测结果的影响,提出应用历史数据研究MIC漂移可能呈现假性升高。

⑷菌株基因型不同。Reynolds等[36]研究发现2001—2007年EMRSA-16从21%降至9%而EMRSA-15从76%升至85%,由于两者MIC有所差异,从而对MIC漂移现象产生影响。Hsieh等[39]通过对我国台湾地区2000—2001年、2004—2005年及2010年124例MRSA标本分析,证实了Reynolds等的观点。

⑸地域差异。我国普遍发现了MIC漂移现象,但国外否定该现象的研究较多,提示MIC漂移现象可能存在地域差异。Kehrmann等[30]对德国两城市进行MIC漂移研究得到不同结果及美国同一时期在不同地域[31-32]进行的两项研究得到的不同结论进一步支持该假设。因此,监测本地区MIC对临床用药有重要指导意义。

4 MIC与万古霉素临床疗效的关系

Choi等[40]回顾性分析了韩国70例MRSA医院获得性肺炎患者万古霉素MIC与临床疗效的关系,采用Etest法进行检测,结果显示:高MIC组(> 1.5 mg/L)早期临床反应率较低MIC组(< 1 mg/L)显著降低(P=0.031),28 d再发感染率明显增高(P=0.038),高MIC是万古霉素治疗失败的独立危险因素。Soriano等[41]进一步证实, 在万古霉素治疗MRSA肺炎时,逐渐增高的MIC和30 d病死率相关。目前有关肺部感染中MIC与万古霉素临床疗效的研究较少,鲜有提示高MIC不增加临床病死率的报道。

2004年Sakoulas等[42]首次发现MIC漂移是万古霉素治疗MRSA脓毒血症失败的重要因素,随后得到许多研究证实。Wang等[43]用微量肉汤稀释法对我国台湾地区123例HA-MRSA脓毒血症标本进行分析发现:治疗14 d后高MIC组较低MIC组病死率增加了16%,治疗30 d时病死率增加了22%。究其原因可能与高MIC菌株较难达到有效的AUC/MIC、存在hVISA、不同的基因亚型及基础疾病较重相关,而高MIC菌株基因型中SCCmec Ⅲ、spa type037所占比例超过80%。该作者进一步研究[44]发现hVISA和病死率无明显关系,高MIC及hVISA均和持续性MRSA脓毒血症明显相关。Jacob等[45]对2000—2010年共计2 439例患者资料行Meta分析发现,敏感折点以内的高MIC(BMD法,MIC≥1 mg/L; Etest法,MIC≥1.5 mg/L)与治疗失败及病死率的增加密切相关。Mavros等[46]行Meta分析也支持高MIC(MIC在1~2 mg/L)与较高的病死率相关。然而,一些研究结果相反。Yeh等[27]对我国台湾地区163例MRSA脓毒血症患者研究发现年龄、肝硬化、诊断48 h内入住ICU为增加病死率的独立危险因素,但并未发现高MIC引起病死率增加的现象。Park等[47]对94例MRSA脓毒血症患者进行研究,也未发现高MIC值(2 mg/L)引起病死率增加,而引起血液感染的原始感染部位及细菌清除情况和30 d内病死率相关。

Price等[48]前瞻性分析显示:高MIC(≥1.5 mg/L)者3个月内死亡风险明显升高,进一步研究发现EMRSA-15及EMRSA-16为该医院主要菌株,不除外菌株不同对于MIC与临床疗效产生影响,但该研究病例数较少。

关于MRSA的万古霉素MIC与临床疗效的关系尚无定论,支持MIC漂移是万古霉素治疗MRSA感染失败重要因素的文献较多。年龄、肝硬化、诊断48 h内入住ICU、菌株基因型、原始感染部位可能为影响病死率独立危险因素。目前研究主要集中在脓毒血症及肺部感染方面,多为小样本回顾性研究,未对万古霉素谷浓度水平进行说明。包括不同地域、不同感染部位的大规模前瞻性研究为临床所急需。

5 结语

MRSA感染一直是临床棘手问题,临床上也出现了耐万古霉素菌株,有研究提出MIC漂移现象,试图以此来解释耐药现象。通过文献综述,发现MIC漂移现象存在与否还有争议,可能受检测时间、分析方法、菌株基因型及地域差异等诸多因素影响,尚缺乏不同检测方法对MIC漂移现象影响的研究。本文同时回顾了MIC与万古霉素临床疗效的相关文献,大多数支持MIC漂移是万古霉素治疗MRSA感染失败的重要因素,这为明确MIC漂移现象提出了临床需求。目前研究多为回顾性小样本研究,还需要进行更多的包括不同地域、不同感染部位的大规模前瞻性研究,以明确MIC漂移现象并来指导临床治疗。
