自噬在脓毒症发生发展中作用的研究进展

脓毒症(sepsis)是感染引起宿主反应失调的严重临床综合征,最终可导致器官功能障碍[1]。据统计,全球每年脓毒症患者达数百万,且死亡率超过25%,仍是重症监护病房中导致患者死亡的主要原因[2-3]。目前大量的研究认为其发病机制与过度炎症反应、氧化应激、线粒体功能障碍、免疫功能紊乱、缺血-再灌注损伤及其机体天然屏障破坏等多因素相关[3-4]。近年来发现,自噬(autophagy)在脓毒症中活性显著升高,可通过调节炎症反应、免疫反应、清除受损的细胞器及其维持细胞内稳态等多方面发挥重要的保护作用[5-7]。本文通过阐述自噬与脓毒症病理生理机制的关系,了解自噬在脓毒症发生发展过程中的分子调控机制,为其临床救治提供新的治疗靶点。

1 自噬的过程及其功能

自噬是真核生物普遍存在的自稳机制,最早是在1962年由Ashford和Porter提出,以肝细胞内发现的自食现象而命名[8-9]。根据其包裹和运输方式的不同,自噬可分为三种类型:大自噬(macroautophagy),小自噬(microautophagy)和分子伴侣介导的自噬(chaperon mediated autophagy),其中大自噬是最普遍的形式。自噬的过程通常分为诱导、形成、转运融合、降解4个步骤。当细胞受到诱导信号后,在胞浆中形成自噬泡(autophagic vacuole),随着不断延伸,将胞浆内受损或老化的线粒体、内质网等细胞器包裹并封闭形成球状的自噬体。最后,成熟的自噬体和溶酶体结合形成自噬溶酶体。在自噬全过程中均需自噬相关基因(autophagy-related gene,Atg)参与,目前发现已有30余种成员,包括ULK1(Atg1)、Beclin-1(Atg6)、Atg5-Atg12/Atg16L1复合物以及LC3(Atg8同源体)等[10]

自噬是受到精确调控的过程,在细胞增长、发育和内环境稳态等方面表现出多种生物学意义,并与人类疾病存在着密切的联系[11-12]。首先,在饥饿、营养缺乏等情况下,是细胞对应激反应的一种适应性机制;其次,自噬是机体重要的抵抗病原微生物的通路。大量的免疫信号可调节自噬,而自噬对免疫和感染也可反过来产生多种效能,包括激活固有免疫和获得性免疫、维持免疫细胞稳态、降解微生物等[7]。除此之外,自噬还和调节生长发育、控制细胞死亡及癌症等密切相关。

2 自噬与脓毒症的发生发展过程

Takahashi首次在盲肠结扎穿孔术(cecal ligation and puncture,CLP)动物模型中观察到肝脏细胞的自噬活性明显升高,而随后在脓毒症死亡患者尸检中也发现肝脏自噬空泡的大量聚集[13]。通过近年来对自噬的深入研究,发现在脓毒症的肺脏、肾脏、心脏、脾脏、脑等各脏器细胞中均存在自噬现象,并证实与脓毒症发生密切相关[5, 14]

脓毒症早期可表现为炎症因子风暴,晚期则出现免疫麻痹,而自噬活性在脓毒症发生发展过程中也出现明显的改变,早期活性显著升高,后逐渐出现衰减。动物实验发现,CLP术后4~6 h,小鼠肝脏细胞和心肌细胞的LC3-Ⅱ:LC3-Ⅰ比例(自噬发生的标志物)明显升高和自噬体聚集,而在CLP后8 h就出现下降[15]。Takahashi等[16]在脓毒症小鼠的心脏、肝脏和脾脏中也观察到类似的现象,自噬活性在6 h达到高峰,而24 h后出现逐渐降低。因此,有学者提出在脓毒症中,自噬活性在短暂升高后,出现明显抑制,而后期的这种抑制作用可导致免疫细胞的凋亡、免疫麻痹,对脓毒症动物有致命的影响。也有学者认为,自噬抑制可能的机制是早期高动力状态下,自噬诱导过程中自噬前体的大量消耗,导致耗尽[14]。支持这一观点的研究有,在脓毒症肺损伤小鼠中,通过过表达LC3-Ⅱ型蛋白能恢复自噬的活性,减轻肺损伤及提高存活率[17]。但是,目前相关研究仍然较少,具体机制有待进一步探索。

3 自噬在脓毒症中的调控机制

业已证实,细胞饥饿是一种启动自噬的有力的生物学行为,主要通过抑制哺乳动物雷帕霉素靶蛋白(mammalian target of rapamycin,mTOR)激酶活性,从而使Unc-51样激酶1(Unc-51-like kinase 1,ULK1) 激酶激活,诱导自噬的发生。当致病微生物入侵机体时, 也能通过营养消耗、激活Toll样受体(Toll-like receptors,TLRs)信号通路等多种途径诱导自噬的产生[8]。研究发现,TLRs激活的自噬髓样分化因子(myeloid differentiation factor 88,MyD88) 或TLRs/IL-1受体功能区介导的IFN-β(TRIF)与Beclin-1相互作用所调控,TLRs与配体的结合诱导这种相互作用,因而降低了Bcl-2与Beclin-1的结合,解除Bcl-2与Beclin-1的抑制,促进自噬的发生[18]。因此,自噬是TLRs激活后的一种效应机制,也是机体重要的抵抗病原微生物的通路。

在脓毒症中,TLRs激活能诱导自噬的发生,而自噬又对炎症反应有调控作用,促进自噬溶酶体的形成,提高病原物的清除效率,加快病原物的清除[19]。研究证实,自噬可抑制过度炎症反应,缺失Atg16L1的巨噬细胞促进内毒素诱导的IL-1β和IL-18的分泌,该信号通路依赖于TRIF依赖性caspase-1的激活[20]。众所周知,NF-κB是调控炎症反应的关键因子,自噬调节蛋白RUBICON能够通过抑制B淋巴细胞瘤10(BCL-10) 复合体的形成而下调NF-κB的活性[21]。动物实验也表明,在脓毒症脑病中,通过抑制NF-κB信号通路,可促进自噬流,有利于神经细胞的生存和修复[22]。此外,自噬还能够通过清除受损线粒体抑制IL-1β、IL-1α、IL-18等的产生[23-24]。反之,抑制自噬可升高炎症因子的水平,增加脓毒症小鼠病死率,机制可能是未能清除受损或异常的线粒体,从而激活了NLRP3炎性复合体[25]

自噬对脓毒症状态下固有免疫和适应性免疫均有调控作用,参与抗原呈递,影响T淋巴细胞的分化,而T淋巴细胞可表达自噬基因并存在自噬现象[26-27]。T细胞受体(T cell receptor,TCR)及CD28共刺激信号可激活自噬,促进T淋巴细胞增殖并发挥效应。反之,特异性敲除Atg12和Atg5可抑制MHCⅡ类抗原向CD4+T细胞呈递;而敲除T淋巴细胞Atg5、Atg7、Atg3及Vps34基因导致脾脏及淋巴结T细胞数量减少,凋亡水平增加[27]。Lin等[28]采用特异性Atg7敲除小鼠行CLP术后,发现小鼠病死率和其脾脏T淋巴细胞凋亡显著增加。除此之外,T淋巴细胞在发育过程中需要通过自噬来清除胞内过多的线粒体及内质网等来维持稳态。自噬机制还可调节T淋巴细胞能量代谢,阻断自噬可抑制细胞内ATP产生[27, 29]

4 自噬障碍与脓毒症的发生发展过程

研究者首次在急性胰腺炎中提出了自噬障碍的概念,认为腺泡细胞的自噬泡聚集是由于其最终阶段受到内毒素血症抑制,导致了自噬体和溶酶体融合障碍[30]。在脓毒症早期,就可过度地激活内质网应激,引起线粒体功能障碍,而自噬障碍与线粒体功能障碍密切相关,可减弱细胞清除细菌和炎症信号蛋白的能力,引起感染和炎症调节紊乱,最终导致器官功能障碍。在脓毒症小鼠的骨骼肌中发现,自噬和线粒体功能障碍均明显增加。而在脓毒症大鼠的心肌细胞中观察到,自噬泡数量增加而自噬溶酶体减少,表明存在自噬障碍(自噬泡形成增多而降解减少),而雷帕霉素诱导自噬后,增加完整的自噬,可减轻心肌损伤[31]。此外,特异性敲除Atg7基因小鼠,TNF-α刺激后可促进肝细胞死亡,减少ATP产生,表现为脓毒症肝功能障碍;而敲除Atg16L1基因可增加脓毒症的炎症反应,促进IL-1β和IL-18等促炎因子的分泌[32-33]。因此,在脓毒症状态下,缺失Atg基因可导致自噬障碍,引起线粒体异常蓄积或功能障碍,促进炎症反应和活性氧物质的大量产生,导致细胞凋亡或坏死[34]

5 激活自噬在脓毒症中的作用

近年来,大量研究证实促进自噬对脓毒症的组织和器官损伤有保护作用。在脓毒症小鼠中,增强自噬能够有效缓解组织器官的损伤,而抑制自噬会增强炎症因子水平和增加病死率[35]。在脓毒症大鼠中,肾小管上皮细胞LC3-Ⅱ呈现早期短暂的升高,自噬体主要聚集在近端小管区,增强自噬后可减轻近端小管损伤[36]。Krakauer等[37]应用雷帕霉素激活自噬可抑制IL-1β和IL-23的分泌,并且缓解金黄色葡萄球菌肠毒素引起的炎症反应和心功能障碍。在脓毒症中,肠黏膜上皮对菌群失调,维护内环境稳定起着重要的作用,促进自噬可减轻肠黏膜上皮细胞损伤[38]。脓毒症脑病是常见的并发症,最近研究已证实海马区神经细胞的自噬现象,进一步促进自噬后发现,具有一定的神经保护作用[39-40]。有研究者通过药物(如腺苷酸活化蛋白酶激动剂、白藜芦醇、卡马西平等)促进自噬,应用在脓毒症动物模型中,均表明具有一定的脏器保护作用[15, 41-42]。因此,合理地调控自噬,促进完整的自噬流,有利于脓毒症中的细胞存活和器官保护,但是,目前所有研究仍局限于动物实验阶段,有待进一步深入探索,为临床救治提供依据。

6 结语与展望

脓毒症一直以来是危重症医学的热点问题,尽管SSC指南对集束化治疗措施的大力推广,但脓毒症病死率仍高居不下,其原因主要是具体病理生理机制尚未清楚。近年来,自噬机制的大量研究有助于从新的角度阐述脓毒症的发病机制。基础研究证实,脓毒症早期各脏器中自噬活性明显升高,可能是适应性防御机制的启动;随着疾病发生发展,自噬或自噬流受到抑制,影响重要脏器功能;而诱导自噬有利于细胞的修复和生存,对脏器功能有保护作用,并改善生存。但是,目前仍有诸多问题尚待解决,如诱导自噬的时机、如何合理诱导自噬水平、自噬在脓毒症中的具体机制、诱导自噬药物的临床研究及转化应用等诸多问题都有待进一步研究。
