脓毒症患儿载脂蛋白E基因多态性分布研究

摘要: 目的 研究我国儿童载脂蛋白E基因多态性与脓毒症的相关性。 方法 将2011年3月至2012年6月临床诊断为脓毒症患儿纳入研究组,将在复旦大学附属儿科医院就诊的健康体检儿童纳入对照组。所有研究对象均采用高通量熔解曲线分析技术进行了载脂蛋白E基因分型。应用SPSS 16.0版本进行统计分析。组间年龄比较采用Mann-Whitney U秩和检验,组间性别和基因频率比较采用卡方检验,混杂因素采用多因素非条件Logistic回归分析,Hardy-Weinberg平衡采用χ2检验进行分析,以P < 0.05为差异具有统计学意义。 结果 2011年3月至2012年6月期间,共收集研究对象285例,其中脓毒症组88例,15例合并中枢神经系统感染。对照组197例。共检测到4种载脂蛋白E基因型:ε3/ε3、ε2/ε3、ε3/ε4、ε2/ε4。脓毒症组与对照组的基因型分布分别为64.4%、73.1%(ε3/ε3);16.8%、10.7%(ε2/ε3);18.8%、14.7%(ε3/ε4);0%、1.5%(ε2/ε4),脓毒症组ε3/ε3的基因型分布低于对照组 (P=0.047,1-β = 0.334,OR=0.585,校正OR=0.559)。伴发中枢感染的脓毒症患者ε3/ε3基因型分布为33.3%、明显低于无中枢感染的脓毒症患者 (67.1%)(P=0.014,1-β = 0.685,OR=0.245,校正OR=0.275)。 结论 载脂蛋白E基因多态性与儿童脓毒症发生及其中枢感染合并症相关,ε3/ε3基因型对脓毒症及中枢感染合并症的易感性明显低于其他几种基因型。

The correlation between the apolipoproteinEgenetic polymorphisms and sepsis in children

Abstract: Objective To study the correlation between apolipoproteinE(APOE) genetic polymorphisms and sepsis in Chinese children. Methods The inpatients suffered with sepsis were enrolled as septic group and the healthy children from child health division were enrolled as control group. The study of APOE genotypes were carried out by polymerase chain reactions followedahigh-resolution melting curve analysis. SPSS 16.0 statistical software was used for data analysis. Mann-Whitney U test was used to compare the age between the groups. Hardy-Weinberg equilibrium was tested using the Pearson χ2-test. The χ2-test was used to compare gender and the genotype distribution between the groups. The odd ratio (OR) was calculated together with its 95% confidence interval (CI). Potential confounding effects of variables were corrected usingamultivariate unconditional logistic regression model. All statistical tests were two-sided and P < 0.05 indicates statistically significance. Results Amongatotal of 285 children collected from March 2011 to June 2012, there were 88 patients with sepsis and 197 healthy children. In the septic group, 15 septic patients were complicated with central nervous system infection. Four apolipoproteinEgenotypes were identified to be ε3/ε3, ε2/ε3, ε3/ε4, and ε2/ε4. The percentage of each genotype found in patients of the septic group and the control group was 64.4% vs. 73.1% (ε3/ε3); 16.8% vs. 10.7% (ε2/ε3); 18.8% vs. 14.7% (ε3/ε4); 0% vs. 1.5% (ε2/ε4), respectively. The number of patients with the genotype ε3/ε3 among septic patients was significantly lower than that among the control individuals (P = 0.047, 1-β = 0.334, OR = 0.585, adjusted OR = 0.559). The number of patients with the genotype ε3/ε3 among the septic patients with central nervous system infection was 33.3%, which was also significantly lower than that among the septic patients without CNS infection (67.1%). (P = 0.014, 1-β = 0.685, OR = 0.245, adjusted OR = 0.275). Conclusions ApolipoproteinEgenetic polymorphisms were associated with the occurrence of sepsis and central nervous system complications in children. The susceptibility of children with genotype ε3/ε3 to sepsis and central nerve system infection complications is significantly lower than that of children with other genotypes.

脓毒症是一种由感染引起的全身炎症综合征[1-2],临床表现为发热、精神混乱、一过性的低血压、少尿或者无法解释的血少板减少,总体病死率约20%,严重者可发生器官功能衰竭、凝血功能异常、休克。严重脓毒症、脓毒症休克患者病死率高达70%[3],儿童脓毒症病死率约25%[4]。脓毒症的发生发展非常复杂,单纯的抗炎症治疗并不能降低脓毒症的病死率[5]

人们一直在探索影响脓毒症易感性的因素[6-7]。本研究课题组前期研究设计了一个。APOE模拟肽,这个模拟肽模拟APOE N端LDLR结合位点氨基酸序列,研究发现APOE23治疗能够明显降低B组鼠伤寒沙门氏菌诱导的细菌性脓毒血症小鼠血浆LPS水平[8]。国外研究资料表明,脓毒症患者出现重症的比例与患者的载脂蛋白E (APOE) 的基因多态性相关[9]。APOE是一个相对分子质量为34 000,由317个氨基酸组成的一种血浆蛋白质,是血浆极低密度脂蛋白 (VLDL) 和高密度脂蛋白 (HDL) 的组成成分[10]。人类APOE基因位于19号染色体上,有3个等位基因 (E2、E3和E4),6个基因型 (E2/E2、E3/E3、E4/E4、E2/E4、E2/E4、E3/E4)。在对外科术后合并脓毒症患者的研究中发现,表达APOE3等位基因术后脓毒症患者不易发展为严重脓毒症[9]。但APOE基因型差异是否影响脓毒症易感性目前尚未见报道。本研究通过高通量熔解曲线分析技术 (HRM) 对脓毒症及非脓毒症患者进行APOE基因多态性分析,旨在明确不同APOE基因型与脓毒症易感性之间的关系。

1 资料与方法 1.1 一般资料

将2011年3月1日至2012年6月30日期间就诊于复旦大学附属儿科医院临床诊断为脓毒症的住院患儿纳入脓毒症研究组。脓毒症诊断标准参照2005年儿童脓毒诊断标准[11]。对照组采用同期门诊健康体检儿童作为对照组。研究对象排除标准为基础疾病为肿瘤、免疫缺陷、营养不良、代谢病、先天畸形、发育不良、肥胖患者。收集所有入组患者临床资料。研究设计通过复旦大学附属儿科医院医学伦理委员学审核。

1.2 DNA抽提

采集入组患者外周血,EDTA抗凝,3 400 r/min离心10 min,吸取白细胞层,白细胞DNA提取采用天根血液基因组DNA试剂盒 (DP318),操作步骤按照操作手册进行。

1.3 APOE基因型检测

APOE基因型采用高能量熔解曲线分析技术 (HRM) 进行检测 (Roche LightCycler480,罗氏诊断产品有限公司,瑞士)。确认鉴定位点为:APOE 112位点 (RS429358) 和APOE 158位点 (RS7412)。112位点引物为5’CAACTGACCCCGGTGGCG3’ (上游) 和5’GCAGGTGGGAGGCGAGGC3’ (下游),产物大小为184 bp;158位点引物为5’CAGAGCACCGAGGAGCTGCG3’ (上游) 和5’GCACGCGGCCCTGTTCC3’ (下游),产物大小为190 bp。PCR反应体系为10 μL,包含以下物质:50 ng模板DNA、5 μL Roche LightCycler480 High Resolution Melting Master (4909631001)、80 μg/L二甲基亚砜 (DMSO; Sigma); 上下游引物各1 pmol、1.2 μL MgCL2(20 mmol/L)。HRM反应条件如下:预变性:95 ℃10 min;循环体系:95 ℃10 s,63 ℃15 s,72 ℃20 s,45个循环;HRM:95 ℃1 min,40 ℃1 min,65 ℃1 s,95 ℃continuous,Acquisitions 25 per ℃。HRM分析参数:Pre-Melt Slider Settings: 85.58~86.68;Post-Melt Slider Settings: 90~91;Tempreture Shift Threshold: 6。285个样本随机抽取120个HRM检测产物进行基因测序,符合率达99.2%。

1.4 血培养及细菌鉴定

所有患者入院期间的血培养采用全自动血液细菌培养仪 (BACT/ALERT 3D,梅里埃生物技术有限公司,法国) 进行培养,细菌鉴定采用VITEK60系统对菌落进行鉴定。

1.5 统计学方法

应用SPSS 16.0版本进行统计分析。组间年龄比较采用Mann-Whitney U秩和检验,组间性别和基因频率比较采用χ2检验,混杂因素采用多因素非条件Logistic回归分析,Hardy-Weinberg平衡采用χ2检验进行分析,以P < 0.05为差异具有统计学意义。

2 结果 2.1 入组患者基本信息

共有285例入组,脓毒症组88例,男性52例,女性36例,年龄中位数为3个月 (1 d至17岁)。对照组197例,男性108例,女性89例,年龄中位数为4个月 (1 d至12岁),两组间年龄 (Z=-0.703,P=0.483) 和性别 (χ2 = 0.450,P=0.502) 差异均无统计学意义。

脓毒症组基础疾病以肺炎为主 (65.9%),其他基础疾病有腹泻 (8.0%)、新生儿小肠结肠炎 (6.8%)、腹部手术后 (9.1%)、外伤 (3.4%)、尿路感染 (2.3%) 等。脓毒症组中46例患者为严重脓毒症,其中合并中枢感染患者15例 (17.0%),血培养阳性56例 (63.6%),脓毒性休克患者13例 (14.8%),多器官功能衰竭综合征 (MODS) 患者8例 (9.1%)(表 1)。

血培养阳性以凝固酶阴性葡萄球菌为主 (9例),其次为金黄色葡萄球菌,白色念珠菌,鲍曼不动杆菌,肺炎克雷伯菌,大肠埃希菌,肺炎链球菌,阴沟肠杆菌,洋葱伯克霍尔德菌,嗜麦芽窄食单胞菌等。

2.2 APOE基因型分布

共检测到4种APOE基因型 (ε3/ε3、ε2/ε3、ε3/ε4、ε2/ε4),不同基因型在入组患者中分布见表 2。本研究对照组的APOE基因型分布符合Hardy-Weinberg平衡 (χ2= 2.230,P=0.991),提示本研究对照组的APOE基因型分布具有代表性。

脓毒症组与对照组APOE基因型分布差异无统计学意义 (χ2=5.714,P=0.116),但是在现有样本量情况下,可观察到脓毒症患儿ε3/ε3基因型频率 (61.4%) 低于对照组 (73.1%),(P=0.047,OR=0.585,95%CI: 0.343~0.995),此结果差异无统计学意义,但把握度也不高 (1-β =0.334)。剔除性别和年龄等混杂因素的影响,ε3/ε3患儿发生脓毒症的校正OR值为0.559(P=0.036,95%CI: 0.325~0.963)。

2.3 合并中枢感染脓毒症患者APOE基因型分布

合并中枢感染脓毒症患儿APOE基因型分布见表 3。合并中枢感染组和不合并中枢感染组APOE基因型分布差异有统计学意义 (χ2= 6.201,P=0.042),ε3/ε3基因型在合并中枢感染脓毒症组的分布频率 (33.3%) 明显低于无中枢感染合并症的脓毒症组 (67.1%),(P=0.014,1-β=0.685,OR=0.245,95%CI: 0.075~0.796)。剔除性别和年龄等混杂因素的影响,ε3/ε3脓毒症患儿发生中枢感染合并症的校正OR值为0.275(P=0.035,95%CI: 0.083~0.911)。

3 讨论

本研究提示APOE的基因多态性与儿童脓毒症易感性及其在中枢神经系统合并症相关,表达ε3/ε3基因型儿童对脓毒症及中枢神经系统合并症的易感性明显低于APOE其他几种基因型。

脂多糖 (LPS) 是革兰阴性杆菌感染诱发脓毒症的始动因子。LPS进入血流后,形成APOE-LPS脂蛋白复合物,机体通过APOE特异性受体 (低密度脂蛋白受体 (LDL-R)、低密度脂蛋白受体相关蛋白 (LRP) 和硫酸乙酰肝素蛋白多糖 (HSPG)) 介导其在肝脏及脾脏等器官中代谢[12-13]。研究表明,不同APOE基因型与LDLR亲和力不同,APOE2与LDLR的结合力比APOE3减弱50~100倍[14-15]。APOE4虽不影响其与LDLR的结合力,但改变了61位点精氨酸侧链的构象,影响了APOE4蛋白的化学稳定性和热稳定性,容易形成折叠中间体,从而形成熔球结构[16],不同APOE基因型影响LPS代谢可能成为潜在个体化预防及治疗脓毒症研究靶点。

APOE是一种重要炎症因子表达免疫调节因子[17-19],其免疫调节作用存在基因型差异[20-23]。有研究表明,LPS刺激后,表达APOE4/4小鼠巨噬细胞α-肿瘤坏死因子 (TNF-α)、白介素6(IL-6)、白介素12p40(IL-12p40) 表达明显高于表达APOE3/0小鼠及APOE3/3小鼠[20]。外源性APOE3可抑制TNF-α诱导小鼠主动脉内皮细胞促炎因子白介素8(IL-8) 表达和单核细胞趋化蛋白-1(MCP-1) 表达[21-22],APOE4则促进IL-8释放表达[21]。APOE2与APOE4同样表现出促进炎症反应[23]。脓毒症初期表现为免疫细胞释放大量炎症因子,基于APOE3模拟肽降低脓毒症动物模型炎症因子表达作用已得到证实[24]

免疫细胞凋亡是脓毒症发病机制之一,APOE有调节细胞凋亡作用。APOE4可引起神经元/胶质细胞的早期凋亡[25],并通过加速内质网应激增强细胞凋亡[26]。与重组APOE3小鼠的神经元细胞相比,重组APOE4小鼠的神经元细胞在受到机械创伤后,其细胞内Ca2+浓度更高,凋亡速度更快[27]。综合上述研究报道,笔者推测ε3/ε3基因型降低脓毒症的易感性可能与其促进LPS代谢、调节炎症因子表达与调节免疫细胞凋亡有关

脓毒症动物模型发现,APOE4等位基因在抵制神经系统炎症方面比E2和E3弱[28]。APOE4增加了血脑屏障对损伤的易感性[29],最近转基因小鼠的研究发现,表达APOE4小鼠和APOE-/-小鼠通过活化壁细胞上的亲环素A-NF-κB-基质-金属蛋白酶-9(CytA-NF-κB-MMP9) 通路而导致血脑屏障受损,从而使神经元摄取多种血液来源的神经毒性蛋白,微血管退化,脑血流量降低[30]。本研究亦证实APOE3/E3基因型脓毒症患者合并中枢感染的比例明显低于其他基因型。

本研究对照组为健康体检儿童,理论上并不是所有人都有机会患脓毒症,理想的对照组为与脓毒症患者同样暴露条件下的患者,后续研究将选取更严格的病例对照进行研究。总之,研究APOE基因型与脓毒症及其合并症相关性对了解患者疾病转归及对并发症早期干预有着重要临床指导意义。
